how do you get the free HDTV with Verizon’s deal?

Verizon had a special "free HDTV" deal when you ordered their FIOS bundle – how and when do you get the TV? Do you need a special coupon? Anyone with experience in this – answers would be much appreciated!
You get a voucher for the TV in a couple of months or a $200 Best Buy certificate, whichever you choose.
